Surgical developments for Malignant salivary gland tumors
Malignant salivary gland tumors (MSGTs) are typically treated with surgery and additional radiation therapy, but new advancements in ablative and reconstructive procedures have emerged. For less severe tumors located in the oropharynx, larynx, and hypopharynx, transoral surgery using laser or robot is a less invasive treatment option that delivers satisfactory oncological and functional results. Endoscopic transnasal surgery is also an effective approach for certain naso-ethmoidal Malignant salivary gland tumors and those found in the nasopharynx. Endoscopic techniques are also useful for MSGMTs necessitating maxillectomy, allowing for better control of margins. For radical parotidectomy, new reconstructive methods involve masseteric nerve transfer, vascularized nerve transfers, and innovative free flaps for complex defects. Research in the future must determine the most effective reconstructive techniques for specific indications, allowing for personalized, evidence-based approaches for patients.
