This systematic review aimed to evaluate the economic evaluations (EEs) of guideline-directed medical therapies (GDMTs) for heart failure with reduced ejection fraction (HFrEF). A total of 59 EEs were included, with the Markov model being most commonly used. Most EEs in high-income countries showed novel GDMTs were cost-effective compared to the standard of care. Key factors influencing ICERs included model structures, input parameters, clinical heterogeneity, and country-specific willingness-to-pay thresholds.
