Impact of 96-Hour Wireless pH Monitoring on PPI Discontinuation

Ambulatory reflux monitoring off proton pump inhibitors (PPIs) is the gold standard for diagnosing nonerosive gastroesophageal reflux disease (GERD), but the optimal diagnostic metrics and monitoring duration are not well-defined. In a study of 132 patients with inadequate PPI response, 30% successfully discontinued PPI therapy after undergoing 96-hour wireless pH monitoring. The total acid exposure time (AET) of 4.0% was the most accurate predictor of PPI discontinuation. Monitoring for 96 hours was more effective than 48 hours in predicting successful PPI discontinuation, helping refine diagnostic strategies for GERD management.
