Recommendations for On-Demand and Long-Term PPI Use in Gastrointestinal Disorders

Experts from the Asociación Mexicana de Gastroenterología recommend on-demand PPI use for maintenance in NERD and mild esophagitis. Long-term, standard-dose PPI therapy is advised for erosive GERD (grades B–D), Barrett’s esophagus, peptic strictures, eosinophilic esophagitis, and acid hypersecretion syndromes. Continuous PPIs are also recommended for patients on chronic NSAID/ASA therapy or with high GI bleeding risk. For those taking antiplatelets or anticoagulants, especially in combination, PPI prophylaxis is advised to prevent bleeding. Dosing should be tailored to symptom control and patient-specific risk factors for optimal outcomes.
