Optimizing GERD Diagnosis and Treatment: A Clinical Pathway Approach

The Saudi Gastroenterology Association’s guidelines for GERD diagnosis and treatment recommend starting with 6–8 weeks of PPI or P-CAB therapy for patients with classic GERD symptoms, and discontinuing or reducing the dose if symptoms improve. For extraesophageal manifestations, a diagnostic workup is recommended before initiating therapy. Lifestyle changes, including weight loss, avoiding meals before bed, and elevating the head during sleep, should be advised. PPIs are most effective for proven GERD, while P-CABs serve as alternatives. Adjunctive treatments like H2RAs, alginate antacids, and baclofen are tailored based on the GERD phenotype.
