High-Dose Pantoprazole for GERD: Why, What the Evidence Says, and Real-World Insights

Gastroesophageal reflux disease (GERD) affects ~15% of Indians, with diverse symptoms across patient subsets. Management involves antacids, H2 blockers, or PPIs, with pantoprazole widely preferred due to its efficacy and safety. However, the standard 40 mg dose often falls short in extraesophageal symptoms, partial responders, NSAID users, or overweight patients. Guidelines suggest dose escalation, but twice-daily regimens reduce compliance. Dual-release pantoprazole 80 mg once daily may optimise acid suppression and adherence. This review explores the need, supporting evidence, guidelines, and clinician experience with high-dose pantoprazole.
